2012年心外icu工作总结

新年将至,回顾过去一年,心外icu护理工作总结如下:

一. 监护工作量统计

过去的一年里,1月至11月监护例数达700例,其中先心病病

人389例(占55.57%)瓣膜病人139例(占19.86%),搭桥术病人

109例(15.57%),法洛氏四联症病人23例(3.29%),大血管置换术

后病人8例,换瓣并搭桥术后病人7例,双向格林术后病人2例,心

脏肿瘤术后病人2例,右室双出口病人1例,三房心术后病人1例,

其他病人19例。病人年龄跨度大,最小3个月,最大82岁。体重最

小3.5kg,最大106.5kg。2012年开展新业务较多,如:心脏肿瘤术

后病人的护理(属国内罕见病例)。监护难度越来越大,在全体icu

工作人员的共同努力下,保持死亡率为1%的良好成绩,低于全国平

均死亡水平(3%)。

二. 护理质量与安全

过去一年,我科在护理部、科护士长及病区护士长的督促下严

抓护理质量与安全管理,科室认真落实护理部—科室两级网络的护

理质量管理,每周、每月坚持自查、抽查,对自查及抽查的结果及

时反馈,及时主动向护理部上报护理不良事件,在护理部的整改下,

及时改进了护理措施,大大降低了护理不良事件的发生率。特别是

在简化护理文书之后,科室加大了对简化后护理文书书写的规范的

自查,发现问题及时反馈给当事人并改正。此外,根据科室具体情

况实行科学的弹性排班,在重点时段安排加强班,并严格执行分级

新进护士进行每两周一次闭卷考试,大大提高了护理人员的基础护理

同时加强专科技术操作的练习,每两周一次有计划的对科内护士进行

实习生和见习生由具备资格的护士专人带教,并由带教负责人和

科内骨干护士定期开展教学讲座、教学查房和技术操作标准示范,加

强专科理论知识与技能的学习,出科前进行理论和操作考核,圆满完

成了带教计划。

针对科室护理疑难病例、危重症病例,开展查房,全科护士展开

讨论,各抒己见,提高了护士对疑难危重病人的护理水平,同时又将

可能发生的护理问题想在前头,提高了对护理问题的预见性和应对能

力。我科全年无一例护理并发症发生。

根据医院优质护理服务工作方案及护理部优质护理服务工作计

划,结合我科实际,制定优质护理服务实施计划,开展优质护理服务,

把今年的工作重点放在提升基础护理质量及加强专科护理技术上,完

善了科室监护人员人数标准、休假标准、工休座谈会制度、监护室探

视制度及家属对监护室护士满意度调查制度。还召开了生活会,将相

关的精神传达到每一名护士,向大家说明,为什么要开展这项工作,

取得科室护士的认可。全科会议上强调服务的重要性,服务就是我们

最好的产品,每一位病人及其家属都是我们的顾客,优质的服务能带

在过去一年里我科重点加强了控感与消毒隔离工作,指派一名年

资较高的护士专职进行控感工作,每月组织全科护士进行控感理论知

识的学习,所有工作人员严格执行消毒隔离制度和无菌技术原则,按

七步洗手法勤洗手,定期对治疗室桌面、和病房内空气进行细菌培养,

大大提高了空气质量及控感的合格率。

一年来,护理部举行两次护理教学试讲活动,我科均派出多人

次参加试讲,通过积极备战,一人如愿加入教学队伍,其余参加人

员也得到极大地锻炼,全年共有3人参与护理教学工作,涉及到5

门课程。

依据《中华人民共和国合同法》、《中华人民共和国广告法》及有关法律、法规、规章的规定,甲、乙双方在平等、自愿、协商一致的基础上,签订本合同。

第一条:广告发布概况

(一)发布地址和位置: (详见附件一:户外牌刊例图)

第二条:合同价款及支付方式

(一)本合同价款为:本合同项下广告位总发布费用为人民币 元整(¥ 元)。以上费用包括:

1、广告位租金及广告发布、税费等相关费用;

2、广告位基础架的费用;

3、广告牌灯源的费用;

4、广告位照明电费;

5、一切有关电器装置费用;

6、合同期内乙方免费为甲方喷挂两次画面(标准为550灯布、4pass以上喷绘),喷挂的效果样本需经过甲方书面确定认可,之后如甲方需要更换广告画面,喷挂的费用可由甲方承担。

(二)本合同项下的广告发布费用分四次支付,具体支付方式如下:

1、合同签定五个工作日,甲方支付乙方合同金额的20%,计人民币 元整(¥ 元);

2、广告发布期满1个月后五个工作日,甲方支付乙方合同金额的20%,计人民币 元整(¥ 元);

3、广告发布期满3个月后五个工作日,甲方支付乙方合同金额的20%,计人民币 元整(¥ 元);

4、广告发布期结束后五个工作日内甲方支付乙方剩余合同金额的40%,计人民币 元整(¥ 元)。 乙方最迟应在本合同约定的甲方付款期限届满前五个工作日向甲方提供同等金额的符合国家及当地税务规定的发票,否则甲方有权拒绝付款并顺延付款时间,直至乙方提供上述发票日止,同时乙方应按合同约定履行合同义务,不得以甲方未支付合同价款作为抗辩。乙方未及时提供发票导致甲方顺延付款时,乙方应将具体情况的书面说明提交给甲方备案。上述发票如无法分割开具,则乙方应在甲方第一次付款前按合同总价款全额开具提供。

乙方所提供户名、账号均以本合同为准,若有更改,应一个日历日内书面通知甲方,否则若因此而造成的任何损失,均由乙方自行承担。

第三条:双方权利义务

(一)甲方权利义务

1、甲方有权在签订本合同前对乙方进行相关的资格审查。乙方隐瞒或伪造相关资质的构成违约。

2、甲方提供的文件不违反法律规定、不损害社会公共利益、不侵犯他人的民事权利。

3、甲方要求发布的广告如果属于依法必须在发布前由有关行政主管部门对广告内容进行审批的,可向乙方提供关审批文件,批准文件应真实、有效。

4、甲方向乙方提交广告样稿所涉及的他人民事权利的合法使用许可证明。在广告中使用他人名义、形象的,事先取得他人的同意;使用无民事行为能力人、限制民事行为能力人的名义、形象的,事先取得其监护人的同意。

5、在乙方完全无瑕疵履行完合同义务后,甲方按照合同约定支付合同价款。

6、甲方需要撤销广告发布订单,可在广告发布日之前通知乙方。在广告发布日前,撤销广告发布订单甲方不承担任何责任。合同期满前一个月,双方可以就延长本合同事宜进行商议,在同等条件下,甲方拥有优先续约权且乙方不得擅自抬高单价或在同等价格下签与其他方。

7、乙方向甲方发出书面验收通知后,应向甲方书面提供符合附件二:《户外广告牌技术规范》的验收报告,甲方可在十五个工作日内进行外观验收(详见附件三:《户外广告发布要求及验收标准》)。乙方仅通过甲方外观验收的,未向甲方书面提供符合《户外广告牌技术规范》验收报告的,不应认为通过验收。

8、合同履行完毕后广告材料及附加物料归甲方所有。甲方可书面授权乙方处理相关广告材料及附加物料。

(二)乙方权利义务

1、乙方保证合法拥有本合同约定媒体的户外广告发布资格。应在签订本合同时携带公司营业执照原件以供甲方确认,并提供其复印件作为本合同之附件。

2、乙方应要求甲方提交《广告法》规定的广告发布所需要提供证明文件。

3、乙方应依法向相关管理部门申请办理本户外广告发布许可审批手续。乙方应在依法办妥户外广告发布许可审批手续之日起日内,发布本合同约定广告。

4、在完全无瑕疵的履行完乙方的合同义务之后,乙方依本合同有权要求甲方支付本合同约定的相关费用。

5、乙方应建立广告审查制度,配备合格的广告审查人员,依法审查广告样稿。广告样稿必须经广告审查人员签字同意后方可发布,并记录在案存档,乙方应对审查结果承担法律责任。乙方应当审核其广告内容,无论从内容、外观、图文等方面均应符合有关法律法规以及有关部门的规定,若发现其内容不符合广告法的相关规定或侵犯其他第三人权利,应于收到审核画面起一个日历日内书面向甲方提出,收到审核画面时间以乙方签收时间为准,若逾期则每日应向甲方支付总额 2 %的违约金。甲方将签字确认的广告画面交与乙方后,乙方应予以签收并加盖公章,并于签收之日起两个日历日内予以发布。若乙方未按甲方指定的时间发布广告,甲方有权单方解除合同,并不承担任何赔偿责任。未经甲方书面同意,乙方不得改动广告样稿,本合同的相关版权归甲方所有。

6、乙方应当及时维护发布本户外广告的相应设施,确保广告发布设施的正常使用和广告内容发布的正常效果,具体标准为550灯布、4pass以上喷绘,喷挂的效果样本需经过甲方书面确定认可。未经甲方书面认可的发布行为不应被认定为履行本合同乙方项下义务的行为。

7、乙方应当如实向甲方书面提供户外广告发布的数据资料。并承担本广告

8、乙方应建立广告档案管理制度,自本合同履行完毕之日起保存本合同文本两年以上。

第四条:违约责任

(一)在乙方完全无瑕疵履行合同义务之后,甲方未按照约定付款,每逾期一天按照未支付金额的 同期存款利息支付违约金。

(二)因乙方原因造成广告无法按期发布的,甲方有权要求乙方顺延履行,每逾期一天并由乙方向甲方支付合同总价款1%的违约金。因发布延迟给甲方带来的所有损失,甲方有权要求乙方全部赔偿。如延迟发布无法实现合同目的的,甲方有权解除合同,并且不影响甲方要求乙方支付违约金及赔偿损失的实现。

(三)乙方未尽法定的广告审查义务导致发布的广告违法,乙方应按合同总金额的30%向甲方支付违约赔偿金。

(四)乙方未尽法定和约定的审查义务,而导致甲方误提供证明材料造成广告作品侵犯第三方合法权益的,乙方承担全部责任。

(五)乙方应负责甲方广告媒体的版面以及灯光等设施的损坏维修和日常保养。乙方应于损毁事件发生时或接到甲方作出修复通知的一个日历日内完成修复受损的广告面等;同时,乙方应当保持广告媒体构架及喷绘广告画面的完整无缺、美观清洁,并符合甲方要求的标准;乙方未能在故障发生后的一个日历日内修复发生故障影响广告发布效果的广告设施和画面内容,应按照发生故障的广告日期数每个日历日向甲方支付合同总价款1%的违约金,并赔偿甲方遭受的所有损失。广告发布期内,乙方应保证广告牌不间断发布,七日以上中断发布且乙方未出具书面合理的理由(如:不可抗力导致),构成根本违约,甲方有权解除合同,并且不影响甲方要求乙方支付违约金及赔偿损失的实现。

(六)因乙方未尽法定和约定的审查义务造成发布的广告规格、内容错误的,乙方应按合同总金额的30%向甲方支付违约赔偿金。

(七)乙方违背本合同,致使本合同被解除或本合同全部或部分不能履行的,应支付甲方合同总金额20%的违约金。

(八)未经甲方书面同意,乙方对本合同甲方提供的资料、信息(包括但不限于商业秘密、技术资料、图纸、数据、以及与业务有关的客户的信息及其他信息等)负保密责任,不得向任何人披露上述资料和信息。

(九)乙方未经甲方书面同意不得擅自把本合同广告位售于其他方,如乙方将广告位售予第三方造成甲方不能按本合同约定发布广告,乙方应支付甲方合同总金额20%的作为违约赔偿金。

(十)本合同的保密及违约条款具有独立性,不受本合同的终止或解除的影响。

第五条:其他约定

(一)因重大社会活动或事件等特殊原因致使广告无法按约定发布的,可由甲乙双方另行补充协议协商解决。

(二)在合同履行期间,乙方因为道路、市政或广告媒体所在建筑物需要进行维修或者其他任何非不可抗力原因必须暂停发布广告时,应当在事件发生后的一个日历日内以书面形式通知甲方,告知暂停发布广告的原因、时间及恢复时间。乙方须于广告发布期限满后补偿因停止发布相应的时间,若无法逾期补足发布天数,或者发布时间具有特定性,逾期发布对甲方已无商业价值,双方以实际发布

(三)在本合同履行期内,经双协商一致,可以解除合同,在解除合同的协议达成之前,原合同仍然有效,双方应严格履行。

(四)本合同未尽事宜,双方可以通过友好协商,另行达成补充协议,补充协议与本合同具有同等法律效力。

第六条: 不可抗力及其他

(一)在合同履行期限内,如遇政府部门将该广告位列入拍卖或广告表达形式、大小尺寸改变,双方以实际发布时间计算广告费,乙方应将多收款退还甲方。同时,乙方有义务协助甲方重新获得该广告位,并应向甲方书面汇报相关工作的具体情况和协助效果,未做出书面汇报导致甲方未重新获得该广告位的构成根本违约;如该合同实际发布期未满一个月因政府原因被拆,则乙方需在事件发生后的一个日历日内返还所有预付广告款。

(二)本合同所指的不可抗力系指不能预见、不能克服、不能避免的客观情况,对不可抗力的认定应以我国有关法律规定为准。

(三)由于不能预见、不能避免和不能克服的客观事件,致使本合同不能履行或者不能完全履行时,遇到上述不可抗力事件的一方,应立即书面通知合同其他方,并应在不可抗力事件发生后十五工作日内,向合同其他方提供经不可抗力事件发生地区公证机构出具的证明合同不能履行或需要延期履行、部分履行的有效证明文件。由合同各方按事件对履行合同影响的程度协商决定是否解除合同、或者部分免除履行合同的责任、或者延期履行合同。遭遇不可抗力事件一方未履行上述义务的,不能免除违约责任。

第七条:争议解决方式

本合同履行过程中发生争议,由双方协商解决,协商不成的应向甲方所在地人民法院提起诉讼。

第八条:其他

本合同一式肆份,甲方执叁份,乙方执壹份。具有同等法律效力。
